Travel Abroad with ICV - WHO International Certificate of Vaccination
International Certificate of Vaccination or ICV is the yellow passport-size document that is now commonly seen on social media. This document serves a very important purpose for OFWs and travelers because this justify vaccines that are already received by the person having this ICV document. This document and the full details is provided at Bureau of Quarantine of the Philippines website, while some highlights are stated in this blog.
ICV serves as a proof of person vaccination that is truthful and acceptable for several countries that are members of World Health Organization, however, this is not mandatory document for international travel. Part of the ICV is shown below,

On this part, the personal data of the ICV is filled where some of the ICV owner has Passport number already indicated while other don't have. As part of the requirement to have this document are the Valid Identification Card or IDs, Vaccination Cards or Documents, and Passport. For ICV acquired starting August 20, 2021, this document will be issued with corresponding Passport number already.

This can be acquire from Bureau of Quarantine offices nationwide, that you can have first an appointment on this website www.icv.boq.ph.

Internal Certificate of Vaccination - ICV costs 300php and 70php for convenience fee.
For person with conditions, Senior Citizens, and PWDs can have Authorization letter with two Valid ID's and medical certicates incase they are not able to attend the said appointment.
An authorized individual of applicants is limited up to two transactions.
Transaction that fail to comply with the "Terms and Conditions" provided can have forfeited payments so please be careful.
